PRIDE, or the Pittsburgh Regional Initiative for Diabetes Education, is a program that was formed by the University of Pittsburgh Diabetes Institute (UPDI) and the University of Pittsburgh Medical Center (UPMC). The PRIDE program uses a multi-faceted approach to provide comprehensive diabetes treatment and care throughout southwestern Pennsylvania.
The PRIDE program redesigns diabetes care plans based on the Chronic Care Model, a treatment framework that has been proven in clinical studies to significantly improve diabetes management, quality of life and knowledge. The Chronic Care Model helps to create more informed individuals who take a more active role in managing their diabetes and develop a prepared, proactive health care team that works with patients towards healthier, more improved lifestyles.
